  2. INVENTARIO • De acción directa: adrenalina, noradrenalina dopamina, dobutamina salbutamol, terbutalina, salmeterol fenilefrina • De acción indirecta anfetaminas • De acción mixta efedrina

  3. ADRENALINA • Farmacocinética: vías sc, im, iv, traqueal. t1/2 corta  infusión • Usos: Shock anafiláctico, cardiogénico, RCP • RAMs: HTA, arritmias, angina, vasoc. pulmonar, extravasación  necrosis • Presentación: amp 1mg/1ml • <0,05 µg/kg/min: 2 >1; >0,05 µg/kg/min: 1

  4. NORADRENALINA • Farmacocinética: iv, t1/2 corta • Usos: Shock séptico, cardiogénico refractario • Presentación: amp 4mg/4ml • RAMs: HTA, arritmias, angina, vasoc. pulmonar, isquemia, extravasación  necrosis

  5. EJERCICIO

  6. DOPAMINA • 1-3 µg/kg/min: D1:  flujo renal 3-10 µg/kg/min: 1, descarga NOR:  Q >10 µg/kg/min: 1: vasoconstricción • PK: iv, t1/2 corta • Usos: IC sx bajo Q; shock cardiogénico, séptico; prevenir / revertir insuf renal ?? • RAMs: HTA, arritmias, isquemia • Presentación: amp 200mg/5ml

  7. DOBUTAMINA • INO +, CRONO +,  Q, RVS , PAM , menor  FC y VO2 • PK: iv, t1/2 corta • Indicaciones: IC sx bajo Q sin hipotensión, shock cardiogénico (+ vasopresores) • RAMs: arritmias, isquemia

  8. DOBUTAMINA Presentación: amp 250mg/5ml Do: 2-15 µg/kg/min

  11. AGONISTAS B2 SELECTIVOS • Representantes: t1/2 corta: salbutamol, terbutalina t1/2 larga: salmeterol, formoterol • Selectividad no es absoluta • Efectos: bronquios cardiovasculares metabolismo glucosa músculo, utero

  12. AGONISTAS B2 SELECTIVOS • Usos: asma EPOC Uteroinhibidores (trabajo de parto pretérmino) Hiperkalemia • RAMs: taquicardia - isquemia temblor hiperglicemia, hipokalemia

  13. AGONISTAS B2 SELECTIVOS Presentaciones Salbutamol: IDM x 100µg Do: 2 puff c/4-6h jbe 2mg/5ml, tab 4mg Terbutalina: amp 0,5mg/1ml tab 2,5mg Do: 1tab c/6-8h jbe 0,3mg/ml sln para nebulizar

  14. AGONISTAS B2 SELECTIVOS Presentaciones Salmeterol: IDM x 25 µg IPS x 50µg Do: 50-100µg c/12h Formoterol: cap 12 µg IPS Do: 12-24µg c/12h

  15. AGONISTAS 1 SELECTIVOS • Fenilefrina, oximetazolina • Efectos: ojo mucosa nasal C V:  RVP, PA, bradicardia genitourinario • Usos: hipotensión durante anestesia raquídea, descongestionante nasal • RAMs - contraindicaciones

  16. ANFETAMINAS • Acción indirecta • Estimulante SNC:  alerta, humor, actividad física,  fatiga, sueño, apetito • CV:  Q, PA • RAMs: confusión, alucinaciones, arritmias, HTA, angina • Dependencia y tolerancia • Interés toxicológico

  17. OTROS SIMPATICOMIMETICOS • Efedrina: mixto, agonista  y β SNC: estimulación CV : FC, Q, PA Respiratorio: broncodilatación Usos: hipotensión anestesia raquídea

  18. OTROS SIMPATICOMIMETICOS • Metilfenidato: estimulante leve SNC efecto mental > efecto motor Uso: SHDA • Modafinil:psicoestimulante no anfetamínico Inhibe recaptación NOR, dopamina Uso: narcolepsia
