  1. Physiologic anatomy of bladder • Two major parts 1) Body ; in which urine collects 2) Neck ; funnel shaped connects with the urethra • the smooth muscle fibers of the bladder is called detrusor muscles

  2. Contraction can increase the pressure in the bladder to 40 to 60 mm Hg Trigone lies on posterior wall It opens into the posteriorurethra Internal sphincter External sphinter

  3. Innervation Of Bladder Cord segments S-2 and S-3 -----sacral plexus-----pelvic nerves----both sensory nerve fibers and motor nerve fibers Sensory fibers detects the degree of stretch Motor nerve fibers are parasympathetic fibers

  4. Skeletal motor fibers through the pudendal nerves---- external sphincter L 2 segment of the spinal cord ----- hypo gastric nerves ---- sympathetic innervations ------ blood vessels

  5. Ureter The two ureters are muscular tubes that extend from the kidneys to the posterior surface of the urinary bladder The urine is propelled along the ureter by peristaltic contractions Each ureter measures about 25 cm long

  6. Micturition • It is the process by which the urinary bladder empties when it becomes filled. • First the bladder fills progressively until the tension in its walls rises above a threshold level • The second step is a nervous reflex called the micturition reflex that empties the bladder or if this fails at least causes a conscious desire to urinate.

  7. The micturition reflex is an autonomic spinal cord reflex, it can also be inhibited or facilitated by centers in the cerebral cortex or brain stem.

  8. Abnormalities • Atonic Bladder Caused by Destruction of Sensory Nerve Fibers. • Automatic Bladder Caused by Spinal Cord Damage Above the Sacral Region. the sacral centers so excitable that even a small quantity of urine elicits an uncontrollable micturition reflex promoting frequent urination.
