  2. Retirees who are making changes must return their Benefit Election Form no later than 5:15 PM on Friday, October 26. Forms can be returned via mail, fax, or email. • All retirees adding a child to their plan must provide documentation establishing proof of eligibility – such as birth certificate, adoption document, etc. Documentation is due to Human Resources NO LATER than December 3, 2012 or dependents will be dropped from the benefit plan. • No action is required if you are not making any changes to your health plan for 2013.

Highlights for 2013 • Two Health Plans • BCBS PPO85 and PPO75 • Plans and premiums remain the same • Continuing Wellness Initiative • Know Your Numbers • Walgreens Prescription Drug Plan is now Catamaran. Prescriptions plans and premiums will remain the same. New Prescription ID cards will only be mailed to those employees and/or dependents who are enrolling for the first time. • BCBS Blue Points will end December 30, 2012. Redeem your points between now and December 30, 2012.

PPO85 2012 Health Plans at a Glance PPO75 Preventive care covered at 100% Co-Pay Office Visit - $30 Specialist - $45 Urgent Care - $45 Emergency Room - $100 (Waived if admitted) Calendar Year Deductible $1250 Single, $2500 Family Individual deductibles apply Medical Cost Sharing Wake County pays 75% Retiree pays 25% Retiree Out of Pocket Maximum $3500 Single, $7000 Family • Preventive care covered at 100% • Co-Pay • Office Visit - $20 • Specialist - $35 • Urgent Care - $35 • Emergency Room - $100 (Waived if admitted) • Calendar Year Deductible • $500 Single, $1000 Family • Individual deductibles apply • Medical Cost Sharing • Wake County pays 85% • Retiree pays 15% • Retiree Out of Pocket Maximum • $2500 Single, $5000 Family

  5. 2013 Monthly Health Premium Rates *New BCBSNC ID Cards will only be mailed to those retirees who are changing plans or who are adding dependent children for the first time. Note: Rates are for Retirees who are eligible for 100% County-paid benefit

  6. “Know Your Numbers” is a program for retirees and their covered spouses (who are under age 65) to identify health risks factors that could lead to chronic diseases such as diabetes, hypertension, heart disease and obesity. Program Outline Includes: • Biometric screening • cholesterol, glucose, blood pressure numbers • Program Period • Begins November 1, 2012 and ends October 31, 2013. • Covered retirees and spouses (who are under age 65) are being asked to obtain a biometric screening. • Biometric screenings are covered at 100% in the BCBSNC network • If a biometric screening is not completed, your insurance premium will increase in 2014 • Additional $20 per month for retiree • Additional $20 per month if spouse is covered

How to Complete your 2013 Enrollment • Review the materials in your 2013 Retiree Open Enrollment Packet • If you are making changes for 2013, complete the Retiree Benefits Election Form and return to Wake County Human Resources via mail, fax, or email • Provide Dependent Documentation (if applicable)
