  2. 棒狀桿菌 • 革蘭氏陽性，不規則型桿菌 • 含異染顆粒，多位於一端 • 無運動性、無芽孢、無夾膜 • 染色抹片中，菌體常呈「人」字形或並排成柵狀。

  3. Corynebacterium diphtheriae白喉棒狀桿菌 Other corynebacteria (coryneform) 伺機感染病原

  4. 致病性 • 白喉棒狀桿菌存在病人或帶菌者的呼吸道、或皮膚傷口中，藉飛沫或接觸傳播。 • 在呼吸道之黏膜寄生繁殖，產生外毒素 (exotoxin)，經黏膜吸收進入血流。 • 局部毒性：引起發炎和黏膜上皮壞死。壞死組織、白血球及凝固血漿蛋白等形成一層灰白色僞膜，阻塞氣道 • 全身毒性：心肌、肝、腎和腎上腺壞死及神經傷害

  5. 白喉外毒素 • 一種A-B毒素 • A為毒力部分，B部分的N端為厭水性，可引導A進入細胞。 • 白喉外毒素可接合多種體細胞。特別是心臟和神經細胞 • 藉影響elongation factor EF-2，使多胜鏈無法延長，抑制蛋白質合成。

  6. 流行病學 • 人類為唯一宿主，在人之間直接傳播 • 好犯5歲以下兒童 • 病人需隔離，有接觸而沒有注射疫苗者，要接受預防性抗生素治療

  8. 臨床症狀 - Respiratory diphtheria • 潛伏期約2-6天，呼吸道開始發炎 (咽喉炎)、低燒。很快出現衰弱和呼吸困難。 • 如果不迅速插管治療或以氣管切開術急救，可能導致窒息。 • 心臟受損引起心律不整。 • 另外視覺受干擾，吞嚥困難、手足癱瘓。 • 死亡多因窒息或心臟衰竭。

  9. 實驗室診斷 • 檢體：鼻腔、咽喉抹子 • 革蘭氏染色：珠狀桿菌＋特異排列方向 • 培養：血液培養基、Löffler’s培養基，或亞碲酸培養基 • PCR 測毒素基因 • 毒力試驗 • 體內試驗：注射培養液於天竺鼠體內(一半預先注射抗毒素血清，另一半沒有) • 體外試驗：Elek平板法

  10. Elek test(瓊脂凝集法) • 接種菌種 • 已知產毒菌株 • 測試菌株 • 已知非產毒菌株 • 含抗毒素濾紙

  11. 錫克氏試驗Schick test • 偵測人體內是否有足夠對抗白喉外毒素抗體 • Intradermal injection of diphtheria toxin • No skin reaction: have NT antibody • Edema + necrosis: no NT antibody, (patient is susceptible to diphtheria)

  12. 治療、預防 • 迅速注射(1) Penicillin或erythromycin，(2) 抗毒素血清 • 維持呼吸道暢通 • DPT三合一疫苗： • 血喉外毒素經甲醛處理去毒 - 類毒素 • diphtheria toxoid + pertussis + tetanus toxoid = DPT疫苗
