With additional Pop up Aid Failure Mode Prevention Method & Occurrence Rating

  3. 4th Why / Root Cause Development Developing the 4th Why / Root Cause Select from the other Entries in the Drop Down List of Root Cause’s / Prevention Controls. Enter a Root Cause/ Prevention directly into the cell/select from the drop down list if previously entered. Root Causes/ Preventions are automatically added to the database. To Update/Edit/View the Database: Select ‘Add-Ins’, ‘Database Rankings’ , ‘Potential Failure Cause DBS’ OR ‘Prevention Control DBS’..
