  2. 4th Why / Root Cause Development Developing the 4th Why / Root Cause Determine two Root Causes and the Prevention Method for each Failure Mode A Root Cause can be defined as a Product, Process or Design Weakness that may result in a Failure Enter the Occurrence directly into the cell or select from the possibilities in the drop down options. With additional Pop up Aid Failure Mode Prevention Method & Occurrence Rating

Enter a Root Cause/ Prevention directly into the cell/select from the drop down list if previously entered. Root Causes/ Preventions are automatically added to the database. To Update/Edit/View the Database: Select ‘Add-Ins’, ‘Database Rankings’ , ‘Potential Failure Cause DBS’ OR ‘Prevention Control DBS’..

  4. 4th Why / Root Cause Examples D4 - Developing the 4th Why …Options The 4th Why defines the Root Cause/s of the 3rd Why / Failure Mode Option 1:The Process Sequence Example ‘VALVE MANUFACTURING’ DESIGN/ APPLICATION ASSEMBLY/ TEST RAW MATERIAL MACHINING TAPPING

  5. 4th Why / Root Cause Examples D4 - Developing the 4th Why …Options The 4th Why defines the Root Cause of the 3rd Why Option 2: Any other logical method or sequence, Example ‘BACK ACHE’ NERVES ASSOCIATED ORGANS POSTURE SPINAL COLUMN MUSCLES
