Program Proposal for Meals on Wheels: Subsidized Rides to Congregate Meals To reduce household food insecurity and in doing so reduce hunger in the elderly of Corvallis (NWS-13) Inputs Outputs Outcomes - Impact Activities Participation Short Term Medium Term Long Term • Time • Money • Surveys • Drivers • Vehicles • Advertising • Partnership • Provide subsidized rides via Dial-A-Bus to congregate lunches at the Senior Center • Lower risk of food insecurity in the elderly by addressing both financial & social needs • Administer surveys to gauge hunger and program interest • Qualifying elderly in the Corvallis area • Family of qualifying individuals • Volunteers that want to drive • Potential donors/ agencies that would assist with funding • Stable & adequate funding to ensure program feasibility & sustainability • Increase awareness of program • Increase awareness of the benefits of congregate meals for the target population • Increase attendance at congregate lunches during the week • Increased socialization amongst peers for participants of program • Lower the risk for food insecurity and hunger in the at-risk subset of the aging population (aged 60 and over) of Corvallis Situation Priorities Some elderly citizens don’t qualify for Meals on Wheels home delivery but cannot afford bus fare to the congregate lunch served at the Senior Center and are at risk for food insecurity Provide seniors at risk a subsidized means of transportation to Meals on Wheels congregate lunches to lower their odds of food insecurity Assumptions External Factors • Competing programs • Target population uninterested in participating • Bad weather or mechanical issues inhibiting service • The program will have funding • The target population will be interested & participate • Dial-A-Bus will be willing to participate & partner up Evaluation • See formative and outcome evaluation plans in Part III
