  2. Inhoud Programma Sedativa: acepromazine alfa2-agonisten Opiaten Locale anesthesie technieken bij gebitsingrepen TIVA

  3. Acepromazine • Voor het eerst bij de mens toegepast in 1950 • Sedatie, evt. in combinatie met opioid: Stehnarcose • Vasodilatatie bij hoefbevangenheid • Uitschachten penis voor diagnostiek • Gecombineerd met alfa2-agonisten in premedicatie voor algehele anesthesie • Gecontraïndiceerd bij hypovolemie, excitatie en epilepsie

  4. alfa2-agonisten • Xylazine Sedamun®, Rompun® • Romifidine Sedivet® • Detomidine Domosedan® , Cepesedan®

  5. alfa2-agonisten Indicaties en toepassingen Sedatie voor kleine chirurgische ingrepen, diverse behandelingen (gebitsbehandeling, endoscopie, Rx, verbandwissel etc.) en als premedicatie voor algehele anesthesie.

  6. alfa2-agonisten Bijwerkingen en contra-indicaties • Cardiovasculaire bijwerkingen hypertensie, bradycardie/bradyarithmie • Ataxie, toegenomen huidsensibiliteit • Zweten • Verhoogde urineproductie • Verminderde maagdarmmotiliteit • Tachypneu

  7. Opioïden Registratie voor koliek en voor combinatie met α2-agonisten • Butorphanol • Morfine • Buprenorfine • Fentanyl • Pethidine • Methadon • Nalbufine • Pentazocine Geen registratie, wel op 6 maanden lijst Geen registratie, niet op 6 maanden lijst alleen toepassen bij niet-slachtpaarden

  8. Opioïden • Indicaties en toepassingen • analgesie • verdieping van de sedatie i.c.m. sedativum • spasmolytisch effect bij koliek • systemische toepassing (iv, im, sc, pleister) • locale toediening (epiduraal, intra-articulair)

  9. Bijwerkingen opioïden Locomotie stimulatie Buprenorfine: Carregaro et al. 2007 Verminderde maagdarmmotiliteit Morfine: Boscan et al. 2006, Andersen et al. 2006 Excitatie/dysphorie Morfine: Mircica et al. 2003 Buprenorfine: Carregaro et al. 2007

  17. totaal intraveneuze anesthesie (TIVA) • Ketamine + midazolam • Thiopental + guaiphenesine • Triple drip

  18. Triple drip anesthesie Voordelen • Geen apparatuur benodigd • Weinig ademdepressie • Weinig cardiovasculair depressieve anesthetica Nadelen • Alleen geschikt voor korte ingrepen (< 1 uur)

  19. Recovery • Vrije recovery • Geassisteerde recovery - Sling systeem - Tilt table recovery - Pool recovery

  20. Postoperatieve pijnbestrijding Voorkomen is beter dan genezen!

  21. de kiesextractie en behandeling van sinusitis Peroperatief: alfa2-agonist, opiaat, ketamine, lidocaïne Postoperatief: NSAIDs
